Athole House is put at number five on a list headed by a B&B in Italy and which also features accommodation in Scotland, the Lake District and Kent.
Owners Josephine and Wolfgang Herrlinger, who have run the B&B for 10 years, are delighted with the award.
Mrs Herrlinger said that what makes the B&B stand out is the quality of personal service.
She said: "Guests arriving by train are met at the station, and at the end of their stay we offer a free luggage drop back to the station in time for their train.
"We will also arrange theatre, cinema or spa tickets, and book the perfect restaurant for a special occasion."
In the reviews of the guest house, it is the breakfasts that stand out.
Mr Wolfgang added: "We have many overseas guests who get tired of a full English every morning, and they really appreciate our alternative offerings, from Craster kippers, Welsh rarebit and many types of omelette to smoothies or freshly made buttermilk pancakes with blueberries."
Athole House has four bedrooms, and is fully booked for most of the year.
One comment on the TripAdvisor said: "I took a short break to Athole House this January and it far surpassed any of the guest houses I stayed in on my trip last year.
"We were made to feel completely at home; the service, quality of the room and the breakfast far exceeded my expectations and any hotel experience I've had previously."
Among its other awards is a five-star rating from the AA and VisitBritain, and a Gold Award from Visit Britain.
